1. Follow this link to access the Adoption Application form.

2. Once open, right click on the Adoption Application form and select "Save As..."

3. In the dialog box that opens, save the PDF form to your desktop.

4. Locate the saved PDF form on your desktop and open it with Adobe Reader.

5. Fill out the form with your applicable information.

6. Save the filled out PDF form with your information to your desktop.

7. Attach and send the filled out Adoption Application form in an email to sweetheartgoldendoodles@gmail.com.
